  2. Complete the Goal setting sheet by choosing 3 SMART goals you want to work towards during these 12 weeks. (this could be weight loss, eat more fruits/veg, & exercise 2 days a week, etc.)
  3. Complete the Readiness to Change Questionnaire to be sure you are in it to win it!
